Back-to-School Checklist
In order to prepare for the start of school in September, all families should submit the following important documents.
Materials due by July 20
• Data Verification Form
• Emergency Contact Card
• Field Trip Permission Slip
• Dining Points Form
• MBTA Pass Form (if needed)
• Hometown News Questionnaire (new students only)
Materials due by August 13
• Health Form and Immunization Records
Please be advised that a student will not be allowed to attend class until the Academy has current health and immunization forms on file.
Required Forms
Medical Release
Submit medical history and current immunization records; required of all students enrolling at the Academy
Emergency Contact Information
Provide contact information in the case of an emergency
Field Trip Permission Slip
Authorize participation in Academy field trips throughout the school year
Dining Points
Purchase dining points to be used with BU Terrier Card; cards may be refilled throughout the school year
MBTA Passes
Enroll in one of two student pass programs
Hometown News Questionnaire
Authorize the Academy and the University to release and publish student achievements and accomplishment
Additional Forms
Appropriate Usage Policy
Agree to the terms of the Academy’s computing and technology policies
SCRIP Order Form
Support the Academy by purchasing gift cards through the SCRIP program. A portion of each gift card sale earns revenue for the BUAPA, all of which goes directly towards annual community events and to deserving student clubs and organizations.
Community Service
Submit service proposals, timesheets, and evaluations
